Request Adult Awards

Currently any award application MUST go through your local association first They will then in turn submit the application forms to the Provincial Association

The average used for ANY award must be an established average (minimum of 21 games or a year book average). Once any bowler establishes a 21 game average they must use their current average and NOT their year book average.

Please download the below spreadsheet and email it to am@otba.ca. You will receive confirmation from the Association Manager.

If you do not receive confirmation, your form was not received.

If you are an Association Manager requesting awards from the OTBA on behalf of the league use this Award Form
